Closing the gap between non profits

How can we create more formal and informal methods of cooperation among nonprofits? We are all working to provide a social safety net of some sort - how can we work together to ensure we are closing the gaps that exist in that safety net.

If I provide overnight services for homeless men and you provide non residential meals how can we be sure that our "cusomers" are aware of each others services, how can we help each other anticipate volume, how can we cooperate with the job placement center down the street?
